What Does My Membership & Commitment Mean?

With a signed Commitment Form, you are pledging to participate in Power of 100 - Ellsworth and make a personal commitment to:

  • Donate $100 twice a year to local organizations (Impact Award Winners) serving the community.

  • Attend two Impact Meetings per year, which are held in April and October. If unable to attend a meeting, you may send a signed, blank check with another attending member to deliver on your behalf. Alternatively, you may mail a check made out to the Impact Award Winner to Power of 100 - Ellsworth, c/o Kaydi Sobottka, 240 North Grant Street, Ellsworth, WI 54011 or provide payment directly to the Impact Award Winner’s Venmo account (if applicable) within seven days of the meeting date. 

  • Understand that if the Impact Award Winner is not a 501(c)3 organization, your $100 donation may not be tax deductible. 

  • Acknowledge that photographs and videos taken at events and meetings may include your image and may be used in promotional material for Power of 100 - Ellsworth.

  • Agree to pay the annual membership fee of $20 to Power of 100 – Ellsworth. Checks can be mailed to Power of 100 - Ellsworth, c/o Kaydi Sobottka, 240 North Grant Street, Ellsworth, WI 54011. Alternatively, the membership fee can be made to the Power of 100 - Ellsworth’s Venmo account @Powerof100Ellsworth. This membership fee covers operating costs and hosted appetizers at the bi-annual Impact Meetings. The membership fee is subject to change yearly.

  • Commit to a one-year membership in Power of 100 - Ellsworth. Should you wish to discontinue membership, you may send an email to PowerOf100Ellsworth@gmail.com

To Become A Member

Fill out the Commitment Form via the link below. The first 100 applicants to complete their Commitment Form AND submit their membership dues will be accepted to the Power of 100 - Ellsworth. 

            

All remaining applicants will be placed on a waitlist and notified when space becomes available.

Frequently Asked Questions

If I can’t attend the event, do I still get a vote? 

No, due to the nature of the voting process you must be present to vote.

 

How do I donate if I cannot attend the meeting?

If unable to attend a meeting, you may send a signed, blank check with another attending member to deliver on your behalf. Alternatively, you may mail a check made out to the Impact Award Winner to the address listed on your Commitment Form or provide payment directly to the Impact Award Winner’s Venmo account (if applicable) within seven days of the meeting date.

 

Can I send my check directly to the selected organization?

The goal of Power of 100 – Ellsworth is to make a large donation on behalf of the whole organization. We’d like each member’s contribution to be part of this donation.

 

Is my contribution tax deductible? 

Checks are written directly to the organization. If the organization is 501(c)3 your contribution is tax deductible. Your canceled check should serve as a receipt for your donation. The organization may also send you a receipt. If the selected organization isn’t 501(c)3, your donation may not be tax deductible.
